For Case Managers

A referral workflow built around your caseload, not around us

Norena is enrolled with our state Medicaid agency as an HCBS Remote Supports provider (Provider Type 36, Specialty SP 756), so referrals move through a process your Case Management Agency will recognize. Most participants access Remote Supports through Community First Choice (CFC), a Medicaid state-plan benefit that doesn't require waiver enrollment or a waitlist, or through an HCBS waiver that already includes Remote Supports as an authorized service.

Before submitting a referral, it helps to have on hand: the participant's current level of care determination, their preferred funding pathway (CFC or waiver, if known), a general sense of daily support needs, and a contact at your agency for follow-up questions. You don't need every detail. The form below flags anything that's missing so we can request it directly.

Coordinated care for discharge planning and beyond

Hospitals, physicians, therapists, and home health agencies refer participants who are transitioning out of a facility, stepping down from more intensive in-person staffing, or simply need a safety layer their current care plan doesn't cover. Norena works directly with your discharge planner or care coordinator to align Remote Supports with existing treatment goals.
